Community Health Needs Assessments

We aim to improve the health and well-being of all people in the communities we serve. Our purpose is to be a national leader in optimizing wellness and equity, and eliminating disparities in our communities. We fulfill our vision by:

• Addressing the social determinants of health

Our health organization is involved in a wide array of community benefit activities and services that need to be categorized and explained - in detail - on the IRS form called "Form 990 Schedule H."

Community Health Needs Assessment (CHNA) Reports

Every three years, Trinity Health conducts a community health needs assessment (CHNA). The CHNA helps communities, organizations and local health systems identify issues that impact health in the community. This process will guide efforts to improve health for the next three years.
